What funding is available in 2024? 4 ways NHS partners can use LivI

1: The Impact and Investment Fund (IIF)

The IIF is designed to help PCNs deliver high quality care to their population. The scheme was also launched to deliver priorities detailed in both the NHS Long Term Plan and in Investment and Evolution; a Five-year GP Contract.

The IIF will be worth £300 million in 2023/24. Funding will be performance based and evaluated on key indicators split into two categories – prevention and tackling health inequalities and high quality care.

Indicators also focus on where PCNs can contribute significantly towards the triple aim. These include:

  • Improving health and saving lives (e.g. through improvements in medicines safety)

  • Improving quality of care for people with multiple morbidities (e.g. through increasing referrals to social prescribing services)

  • Helping to make the NHS more sustainable

2: Modern General Practice Access

The General Practice Improvement Programme (GPIP) provides nationally funded support for practices and PCNs to implement Modern General Practice and realise benefits quickly. This is focused on tackling the 8am rush, providing rapid assessment and response, and reducing the need for patients to ring back another day to book an appointment.

There are three core components which are digital telephony, simpler online requests and faster navigation, assessment and response.

A total £172.2 million will be available and paid to PCNs according to population in 12 equal payments over the 2023/24 financial year which equates to an average of ~£11,500/PCN/month.

  • For the Quality and Outcomes Framework 23/24, this will be approximately £15,800 per practice

  • For local capacity and access improvement, a part or all of a total of £73.8 million will be paid to PCNs based on commissioner assessment of a PCN’s improvement over the course of 2023/24

  • For transition cover and transformation support funding, an average of £13,500 is available to provide additional capacity and smooth the transition to new models

Additional funding is also available for wider digital support. This includes:

  • £90 million to support practices to move from analogue to cloud-based telephony systems available via national frameworks

  • £71 million for digital tools for online consultations, messaging and appointment booking. This will be added to ICB allocations to spend on accredited digital systems via a national digital pathways framework

3: Cloud-based telephony

From 2025, all practices and PCNs must switch to cloud-based telephone systems. These use the internet rather than traditional phone lines, with phone system functions in the cloud. While software and data can be stored and processed on internet-accessed servers. 240m will be available for GP practices to buy and install these systems from 2023-2025.

NHS Digital has developed the better purchasing framework to guide future services and set standards for unified communications and telephony services in general practice and primary care. Practices can also contact NHS England’s National Commercial and Procurement Hub team for further information about this framework and the wider work of the Advanced Cloud Based Telephony programme.

4: Additional Roles Reimbursement scheme (ARRS)

The overall value of ARRS in 2023/24 will be over £1 billion. PCNs are allocated a sum for the entire year, based on their population. Although PCNs are able to claim up to this maximum sum each year, an underspend has commonly been reported.

NHS England will review ARRS further to align it with future general practice goals. Staff under this scheme will be integral to the core general practice cost base beyond 2023/24, and PCNs can provide permanent contracts as needed. PCNs are urged to maintain recruitment, maximising ARRS entitlement.

ARRS roles may include clinical pharmacists, physician associates, nursing associates, paramedics and occupational therapists.
